Welcome to Bexley Diabetes

Bexley Diabetes, as a part of Bexley Health Ltd, are currently responsible for the delivery of the Diabetes Education service to the patients and health care professionals of Bexley.

The current team have been responsible for the delivery of the X-PERT Structured Patient Education Programme since January 2010. We have a strong cohort of X-PERT trainers which include a number of health care professionals and patient champions. These trainers have delivered courses to over 1500 Bexley patients in the 4 years they have delivered the service. As a result of our great team work we have managed to improve the health outcomes of our patients with type 2 diabetes and help them understand their diabetes. The team have won a series of awards to reflect this achievement and most recently won the X-PERT Health Award for “Greatest reduction in cardiovascular disease risk factors.”

We will continue to deliver our high standard of education to patients throughout 2014.

In addition to educating the patients we also have a robust training programme for our local health care professionals. Each year we run a series of education updates and training sessions for your GP’s, practice nurses, HCA’s and other professionals. These courses allow health care professionals to stay up to date with the changing treatments, guidelines and even technologies in regards to diabetes management. This will allow your GP’s to provide you as a patient the best possible diabetes care in practice.

Aside from the provision of education services Bexley Diabetes also create and collect the data from the annual diabetes patient survey, which will allow us to work in partnership with your GP to improve services. We distribute and create the diabetes information packs that you would have received from your GP at diagnosis, we arrange the diabetes consultant led virtual clinics that are held within GP practice and act as the primary signposting agency in regards to diabetes queries for Bexley.
